Motions, Affidavits and Proposed Order

CJ-D 400  Motion - Revised July 2007 You must use bonded paper.
Motion for Counsel fees Pendente Lite This form is used if you are requesting an order for your spouse to help pay for an attorney to represent you in your divorce action.  Instructions included in this form. 
Motion for Alternate Service and Publication

(Must be accompanied by an Affidavit in Support of this motion)

Motions for Alternate Service do not require a certificate of service (page two of the motion).  Also, you are not required to file a proposed order with this motion.  Motion MUST be printed on bonded, acid-free paper.
Affidavit in support of Motion for Alternate Service Must accompany the Motion for Alternate service to state the reasons why you are unable to have the defendant served by sheriff or constable.  MUST be printed on bonded, acid-free paper.
Motion to Exempt from Parent Education You must print both pages of the motion.  The second page MUST BE PRINTED on the back of the first page and you MUST use bonded, acid free paper.
Motion to Impound Address

(Must file an appointment of Agent and Affidavit in support of motion)

If you file this motion, you MUST also file an Appointment of Agent (Domestic Relations) form also.  You must print both pages of the motion.  The second page MUST BE PRINTED on the back of the first page and you MUST use bonded, acid free paper.
Affidavit in Support of Motion to Impound Address Must be filed with the Motion to Impound Address.
Appointment of Agent (Dom.Rel) Must be filed with the Motion to Impound Address
Motion to Release Birth Certificate

(Must be accompanied by an Affidavit in Support of this motion)

If you file this motion, you MUST also file an Affidavit in Support of said motion. Motion MUST be printed on bonded, acid-free paper.
Affidavit in Support of Motion to Release Birth Certificate Must be filed with the Motion to Release Birth Certificate
Affidavit Form Form must be printed on bonded, acid free paper.
Emergency Affidavit Form Must accompany all emergency motions.  Form must be printed on bonded, acid free paper.
Proposed Order  This MUST be filed with all motions, except the Motion for Alternate Service.  Order MUST be printed on bonded, acid-free paper.
